Inland Empire Perinatal Loss Survey

The Maternal Health Network values the voices of pregnant families and as such collects information about their experiences with the maternal health system.

The MHN has launched a new survey. Open until June 2026.

Developed by Diversity Uplifts, Inc., in collaboration with Dr. Kendra Flores-Carter (CBU & The NAEBOR Clinic) and the Data Subcommittee of the Maternal Health Network of SB County, this Inland Empire Perinatal Loss Survey is the first-ever systems-wide quality improvement perinatal loss survey in the field of maternal health, created to better understand the additional supports necessary to enhance services and improve the health and well-being of expectant and postpartum families.

The survey takes around 30 - 45 minutes to complete, and participants will be compensated with a $50 VISA gift card for their time. To maintain the integrity of the survey responses, please DO NOT share flyers or direct survey links on any social media platforms. Gift cards are limited to the first 100 responses. Please reach out to Kia Hines and Dr. Kendra Flores-Carter for additional information about the survey and/or if you’d like to partner with the MHN to disseminate the survey. Physical flyers are available upon request.

Previous Consumer Survey Reports & Briefs

Since 2019, the Maternal Health Network has collected surveys from families who are planning to get pregnant, those who are pregnant, and those who have recently delivered a child to gain a better understanding of their experience in the maternal health system.
