2024 Consumer Survey is Live!

The Consumer Survey is officially live! The Maternal Health Network of San Bernardino (MHN) is collecting data on San Bernardino County residents' maternal health experiences. We are gathering feedback from families who are receiving, or have received maternal health services in San Bernardino County in order to better understand what additional support is necessary to enhance services and improve the health and well-being of those who are pregnant, birthing, and postpartum.

The survey should take no longer than 15 minutes, and participants will be compensated with a $25 Amazon gift card for their time.  Please share the flyer with your families and spread the word to your personal and professional networks. Gift cards are limited to the first 400 responses. Please reach out to Kia Hines and Dr. Kendra Flores-Carter for additional information about the survey and/or if you’d like to partner with the MHN to disseminate the survey. If you would like posters and flyers delivered to your office, please email them as well.
